– Photo IDs of ALL adults in household (usually a license).
– SSN cards for all household members
– Proof of income of all working household members which includes payroll, any type of retirement, alimony, child support, SSI, or fostering income.
– Current Food Stamp letter. (If applicable)
– Custody papers for any children living in your household that you are not the biological parent of.
– Proof of Jacksonville Residence within the last 30 days via a first class piece of mail.
