How Our Utility Assistance has helped
Our Utility Assistance program has helped families, single parents, and people of all ages. If you’re in need of assistance, reach out to us and we’ll try and see how we can assist you.
How It Works
If you’re in need of assistance, there are a variety of ways that we can help. However, it all starts with you reaching out to us. If you need assistance, you can either contact us by phone or email at jaxoutreach36265@gmail.com. Please know, there are a few forms you will need to fill out if you need assistance. This can be accessed by clicking on the buttons below. Be sure to fill them out before any consultation to ensure the best service necessary.
Eligibility Requirements
- Photo IDs of adults in household (usually a license).
- SSN cards for all household members
- Proof of income of all working household members which includes payroll, any type of retirement, alimony, child support, SSI, or fostering income.
- Custody papers for any children living in your household that you are not the biological parent of.
- Proof of Jacksonville Residence within the last 30 days via a first class piece of mail.
- A copy of your most current bill must be provided
